Management of grammatical time marking in post-stroke aphasia: study with French-speaking subjects suffering from fluent and non-fluent aphasia in chronic stage

Résumé de l'étude

This study aims to evaluate the effectiveness of speech therapy (individual or group) focusing on grammatical time marking, targeted at the underlying deficient processes. The main objective will be to assess whether the proposed therapy improves the grammatical time marking of the items worked on in sessions. We will also examine whether the observed progress generalizes to untreated items and to more ecological contexts (related to the subject's daily life), as well as whether the observed progress is maintained at 2 weeks and at 1 month post-therapy. The management will be offered to several French-speaking aphasic individuals (fluent and non-fluent) in chronic stage suffering from difficulties in referring to time through verb inflection. Two modalities of management will be proposed: individual and group. The first part of the study, proposing individual management, will include between four and five participants with brain injuries. The second part of the study, proposing group management, will include four participants with brain injuries (two groups of two participants). The therapy will last 1 month with 3 weekly sessions of about one hour.

Intervention étudiée

The management focuses on grammatical time marking. Whether individual or group, the management aims to work on the underlying processes of grammatical time marking identified as deficient during a preliminary assessment. The intervention consists of 4 steps inspired by models and hypotheses from the literature on the subject. 1) In step 1 "selection of temporal adverbials", the subject must identify the time encoded in sentences (via the temporal diacritic features contained in the inflected verb), then select the correct adverbial from those proposed. 2) In step 2 "selection of inflected verb forms", the subject must translate the time encoded in the sentence (via the temporal adverbials) into temporal diacritic features to select the correct verb form from those proposed. 3) In step 3 "production of inflected verb forms", the subject produces inflected verbs. To do this, they must translate the time encoded in the message (via the temporal adverbials) into temporal diacritic features and retrieve the correct verb form corresponding to the encoded time. 4) In step 4 "semantic narration", the subject describes imagined scenes using inflected verbs according to the previously defined reference time.

Critères de participation
- Must have been a victim, in adulthood, of a stroke (AVC) evidenced by imaging and causing aphasia (fluent or non-fluent). The interval between the stroke and participation in this study must be greater than 6 months. - Must be a native French speaker or have excellent command of French. - Must present disorders of grammatical time marking evidenced by a language examination. Critères d'exclusion
- Must present chronic symptoms of a substance use disorder related to alcohol or drugs (according to DSM-V criteria). - Must present significant uncorrected visual and/or hearing impairments. - Must present significant disorders of oral/written verbal comprehension. (BASEC)

Maximum age: 75 Years
Minimum age: 18 Years

Inclusion Criteria:

- Have had an imaging-objectified stroke in adulthood that resulted in aphasia (fluent
or non-fluent). The time between the stroke and participation in this study must be
greater than 6 months.

- Be a native French speaker or have excellent mastery of French.

- Be between 18 and 75 years old.

- Present grammatical tense marking disorders objectified by language evaluation
(deficit scores on the "Batterie d'?valuation de la Production Syntaxique" (BEPS) verb
flexion task, Monetta et al., 2018; and/or on the "Test d'expression morpho-syntaxique
fine" (T.E.M.F.) active sentence production subtask, Bernaert-Paul and Simonin, 2011).

Exclusion Criteria:

- Present chronic symptoms of a substance use disorder as defined by Diagnostic and
Statistical Manual of Mental Disorders (DSM-V) criteria.

- Have significant uncorrected vision and/or hearing impairment.

- Have significant impairments in oral/written comprehension.

- Present apraxia of speech or a severe arthritic disorder

- Present hemineglect

- Present impaired judgment and discernment, objectified by a neuropsychological
evaluation
