Telerehabilitation after a stroke: A study on the feasibility, safety, and performance evaluation of a digital home exercise program

After a stroke, patients typically spend a longer time in a rehabilitation center. They are then discharged home and are expected to continue their movement exercises on their own. However, it is often difficult to maintain motivation and perform the exercises at home with the same intensity as in inpatient rehabilitation. The digital application "Blended Clinic" provides an innovative platform that includes, among other things, a personalized exercise program for home. This can already be learned during the inpatient stay and continued at home. In our study, we want to find out whether the therapeutic concept with the digital application "Blended Clinic" is feasible, safe, and implementable.

The innovative digital application "Blended Clinic" offers a specific home exercise program for patients after a stroke, which can also be performed from home. In the training module, patients receive a personalized training plan (including videos). In addition to training, the therapeutic concept also includes coaching and monitoring components. Patients are accompanied by a therapist throughout the study period. It is possible through the digital application to ask questions about training or other topics. Additionally, patients receive relevant health information for the target group via messages or audio content, which is intended to inform, educate, and empower them for better self-management. The monitoring serves to control important health parameters, e.g., blood pressure. The data can be entered and stored in the digital application. During the inpatient stay, participants will have three to four appointments. At these appointments, they will get to know the digital application "Blended Clinic", install it on their own device, and receive a tailored exercise program. For this, personal and health-related data will be collected, physical performance will be evaluated, and various questionnaires will be completed. From this point on, participants will independently perform exercises with the support of the digital application. During the inpatient stay, participants will wear a type of watch that measures their activity. Additionally, their blood pressure will be measured daily. From the time of discharge from the rehabilitation clinic, the 12-week training phase begins, which participants will complete in their own environment. During this time, they will continue to perform daily exercises and measure their blood pressure. In the last three weeks of the 12-week training phase, participants will again wear the activity monitor, which they will receive by mail. After these 12 weeks of training, we will invite participants for a new appointment at the rehabilitation clinic. This appointment lasts about two hours, during which physical performance will be re-evaluated, and participants will provide feedback on the feasibility, safety, and performance of the application based on various questionnaires.

- Age over 18 years - Diagnosis of a stroke - Ownership of a smartphone or tablet that has a stable Internet connection and uses Android 7.1 or iOS 14 or higher as the operating system (BASEC)

- Severe speech disorder (aphasia) - Severe visual, neurological, cardiopulmonary, psychiatric, or orthopedic limitations that prevent a participant from following the instructions of the project staff or limit the execution of the exercise program - Recent events such as surgery or other surgical procedures and fractures that limit the execution of an exercise program (BASEC)
