Summary description of the study

Cette étude examine si la prise d'œstrogènes (Estradiol Valérate ou Éthinylestradiol) augmente le taux d'ocytocine dans le sang. L'ocytocine est une hormone liée à la régulation émotionnelle et à l'équilibre hydrique dans le corps. Un déficit en ocytocine pourrait entraîner des symptômes psychologiques tels que l'anxiété et la dépression chez les personnes souffrant d'un déficit hormonal appelé déficit en AVP.

Objectif de l'étude :
Vérifier si la prise d'œstrogènes (Estradiol Valérate ou Éthinylestradiol) augmente le taux d'ocytocine chez des personnes en bonne santé, mais pas chez des patient·e·s avec un déficit en AVP. Cela pourrait constituer un nouveau test pour diagnostiquer un déficit en ocytocine.

Déroulement de l'étude :
Partie 1 (adultes en bonne santé) :
Étude randomisée en double aveugle
Les participants prennent lors de deux journées d'étude soit 6 mg d'Estradiol Valérate (Progynova®) soit 30 mcg d'Éthinylestradiol (Elyfem®).

Partie 2 (patient·e·s avec déficit en AVP) :
Étude ouverte sans aveugle
Les participants prennent soit 6 mg d'Estradiol Valérate soit 30 mcg d'Éthinylestradiol, celui qui a provoqué une augmentation plus forte d'ocytocine chez les témoins sains.

Attente :
On s'attend à ce que les œstrogènes augmentent le taux d'ocytocine chez les adultes en bonne santé, mais pas chez les personnes avec un déficit en AVP.

Intervention under investigation

Partie 1 (adultes en bonne santé) :
Prise de 6 mg d'Estradiol Valérate (Progynova®) et 30 mcg d'Éthinylestradiol (Elyfem®) avec un intervalle d'au moins une semaine (double aveugle croisé).

Partie 2 (patient·e·s avec déficit en AVP) :
Étude ouverte sans aveugle
Prise soit de 6 mg d'Estradiol Valérate soit de 30 mcg d'Éthinylestradiol, celui qui a entraîné une augmentation plus forte d'ocytocine chez les témoins sains.

Criteria for participation in trial

Partie 1 : Témoins sains
1. Adultes en bonne santé comme groupe témoin
2. Pas de médication (y compris les contraceptifs hormonaux)
3. Patientes (sauf post-ménopausées) : cycle régulier (durée de 21 à 35 jours) au cours des 6 derniers mois

Partie 2 : Étude pilote chez des patientes avec déficit en AVP
1. Diagnostic confirmé de déficit en AVP
2. Âge ≥ 18 ans
3. Patientes (sauf post-ménopausées) : cycle régulier (durée de 21 à 35 jours) au cours des 6 derniers mois ou en cas de traitement hormonal substitutif avec une pause d'une semaine de ce traitement

Exclusion criteria

Critères d'exclusion pour les témoins sains et les patientes avec déficit en AVP :
1. Participation à une étude avec des médicaments expérimentaux dans les 30 derniers jours
2. IMC > 30
3. Âge > 50 ans
4. Usage de substances illégales (sauf le cannabis) dans les 30 derniers jours
5. Consommation de boissons alcoolisées > 15 boissons/semaine
6. Consommation de tabac > 10 cigarettes/jour
7. Grossesse et allaitement
8. Contraception hormonale
9. Migraines avec ou sans aura
10. Toute maladie cardiométabolique, cardiovasculaire et hématologique (y compris thrombose veineuse profonde (TVP)/embolie pulmonaire (EP) et thrombophilie)
11. Troubles hépatiques actifs ou valeurs d'alanine aminotransférase (ALAT) ou d'aspartate aminotransférase (ASAT) plus de 2,5 fois la plage normale
12. Maladie rénale chronique diagnostiquée (MRC) > stade III (DFG < 30 ml/min)
